 The bolt fatigued at the root of the last thread in the front cover.



  This is always the case.  All threaded fasteners always fails at the first
(last,
depending on you reference point) thread.  There are exceptions, but this is
a near ironclad rule.  Not quite like "the ball fell straight down when
dropped" but close.
