Duhh -- ok, switch "Default" and "First Page" in the instructions below
& it will work swimmingly.  Note you must change the style of the 1st
page manually to "first page" (right click on page style field at bottom
of window & select fist page).  I went into brain freeze mode while
typing this up.  There is no magic here -- Default and First Page are
just labels for page styles.  The key is you want the first 3 pages to
be some style other than the default.  Note you can add new styles (
e.g., Frontis Material) and do the same process as below to make it all
work out correctly.
